Fort Hudson Care Management has outgrown its space on the Fort Edward campus and moved to downtown Hudson Falls. Twelve full-time care managers and service coordinators occupy the first floor of Bank Commons at 124 Main St.
According to Andrew Cruikshank, Fort Hudson Health System CEO, “care management is the cornerstone to achieving a more integrated, coordinated and centered health care system. Payers of service, including Medicare and Medicaid, are shifting from a volume-based system to a value-based system, which is helping to drive the growth in our programs.”
Fort Hudson Care Management manages care and services for over 300 Medicaid-covered individuals, primarily senior citizens. It covers a vast six county area, including Clinton, Hamilton, Essex, Warren, Washington and Saratoga counties.
Rhenda Campbell, RN, director of Fort Hudson Care Management, sees continued growth ahead.
“As Medicaid continues to move toward managed care, the number of individuals in the community and in nursing homes requiring Care Management services will only increase. Although many of our clients have complex needs, both social and clinical, with the right support at the right time and in the right place, they can maintain their independence in the setting of their choosing. To us, that’s exciting.”
Fort Hudson is now developing a new Care Management model for individuals not able to qualify for Medicaid.
“Navigating the health care system, planning for future care needs, and coordinating services are a challenge for even the savviest consumer. We are called upon daily, and provide support informally now, but see the demand for a more focused and dedicated team to meet this need,” said Campbell.
Fort Hudson intends to launch a private-care management program by the end of the year.
Fort Hudson Health System is a nonprofit multilevel senior health care and housing provider located in Fort Edward.
